        The customer above hereby assumes all financial obligation and guarantees payment with regard to incurred charges on all vouchers for jobs ordered by authorized personnel and guarantees payment of any and all charges incurred on his / her account.  
        
        Cancellation can be made no less than 2 weeks prior to the booking date on domestic services and 3 weeks on international services for a refund. No refund will be made if cancellation has occured within this time frame.  Failure of passenger to show will result in credit card being charged full price. 
        
        Black Dog Limo will contact the customer to retreive Credit Card Information in order to make an authorization before the job is performed. 
        
        The customer above authorizes Black Dog Limo to process the credit card above as "signature on file" for ground transportation services and/or to charge company's credit card in the event of non-payment of bill in 30 days from the date of receipt of bill.  
        
        Printed Name and Date on this Email form represents that the signatory (person) has the legal right, power and authority to enter into this Credit Application and/or approve any credit card transactions that should take place as a result of scheduled services:
